How to read a before & after

The other trick: when the photo is taken

Timing misleads as much as lighting. Walk the real timeline.

Shedding

The transplanted hairs usually fall out — “shock loss”. This is expected; the roots stay. A photo now would look worse, not better.

Before you trust any before & after — ours included — check that it:

  • was shot in the same light, from the same angle and distance, both times
  • shows the hair at the same length and dryness in both photos
  • states a realistic timeline — final results take about 12 to 18 months
  • shows the donor area too, not only the transplanted zone
  • includes more than one angle, not a single flattering crop
  • isn’t combed, producted, or fibre-filled to hide scalp in just one photo
  • is a real patient shown with their consent — not a stock or borrowed image

This isn’t here to make you cynical. A good transplant genuinely regrows hair — real results are real. It’s here so a single pair of photos can’t fool you either way. If a clinic — ours included — won’t control for the things above, that is your answer.

What this tool does not do

It teaches you to read a photo. It cannot read your head. It does not:

  • diagnose your hair loss or its cause
  • tell you whether you are a candidate for surgery
  • predict your result, or how many grafts you would need
  • judge any clinic’s real surgical work from its marketing images
  • replace seeing a result in person, or an in-person examination

What a pair of photographs is actually evidence of

A fair comparison is an attempt to hold everything constant except the hair, so that any difference you see is a difference in the hair. Almost nothing is held constant unless the photographer deliberately makes it so — and the things that drift are not small. Lighting, angle, distance, hair length, styling and wetness each change how much scalp shows through, and each of them can move further than a real early result does.

So the honest reading of any single pair is narrower than it looks. At best it shows one clinic’s chosen case, on somebody else’s head, under conditions that clinic selected. It is not evidence of the average result, and it was never assembled to be. A gallery is a sample chosen by the person being judged.

The demonstration above is not an accusation

Both panels hold the same hair. The strand count does not change between them; the camera, the light and the comb do. Nothing was transplanted and nothing was removed.

That is worth sitting with, because the usual framing of this subject is fraud, and fraud is the less useful half of the problem. Your eye is not being deceived by a forgery. It is doing what eyes do: reading scalp visibility, which is governed far more by light and styling than by follicle count. Which means a careful, honest clinic with careless photography produces the same misleading pair as a dishonest one produces on purpose.

The practical consequence is that intent is invisible in the image. You cannot tell an exaggeration from an accident by looking harder at the photograph. You can only ask what was controlled — which is a question about method, not about pictures.

The limit that no amount of care removes

Suppose a clinic does everything right: same light, same angle, same distance, same hair length, dry in both frames, shown, twelve months elapsed. That pair is now honest, and it still cannot answer the question you came with, because it is a photograph of somebody else’s scalp.

What you would get instead depends on things no image of another person contains: how much donor hair you have and what condition it is in, the pattern of your loss and whether it has stabilised, your age and how much further that loss may travel, your scalp laxity, and the contrast between your hair and your skin. Two people with the same can end up looking different for reasons that have nothing to do with the surgeon — and the arithmetic behind a graft count is a separate question again.

This is the ceiling on the entire genre. A before-and-after can be evidence that a clinic is capable of a good result. It can never be evidence of yours.

The one thing in a photograph you can check against a source

Most of what makes a pair fair is a judgement call — whether the light matches closely enough, whether the angle is near enough to the same, whether the hair is at a comparable length. One element is not a judgement call at all, and it is the one most galleries leave off: when the second photograph was taken.

Transplanted hairs typically shed within days of the operation and may take several months to regrow. Hair around the donor or recipient site can shed as well — shock loss — and this is transient; new hairs typically appear after three to six months, with grafts monitored over the following six to twelve months until they have fully matured. This is the general post-operative timeline and was not established specifically on .[1]

That gives you a fixed point to hold a gallery against: photographic documentation of a result is taken around six to twelve months after the operation.[1] So a pair captioned three months is not showing you a result, it is showing you a stage — and a pair carrying no elapsed time at all has withheld the one detail you could have checked without having to trust anyone.

What we hold ourselves to

The standard above is not one we get to apply only outward. Our own gallery is published with the ways it falls short of it stated on the page: there is no donor-area photograph, no surgeon named against a case, no technique recorded per case, and three of the twelve are under twelve months. We would rather write that than let a reader who has just been handed a checklist discover we fail parts of it.

Frequently asked questions

Are the two photographs in the tool really the same hair?

Yes. The strand count is identical in both panels and is shown on screen. Only the lighting, the camera angle and the styling change between them.

Does this mean before-and-after photos are worthless?

No. It means a single pair is weak evidence on its own. A pair that states its conditions — same light, same angle, same length, a real elapsed time, the donor area included — is worth considerably more than one that states nothing.

Can I tell whether a clinic is being dishonest from its photos?

Usually not, and that is the point. Careless photography and deliberate exaggeration produce the same image. What separates them is what the clinic controlled and is willing to tell you, which is a question you have to ask rather than something you can see.

Why do so many galleries show only the top of the head?

Because the top is the half that improved. The donor area at the back and sides is where over-harvesting shows, and it is absent from most galleries in this industry.

If a clinic’s photos look good, does that predict my result?

No. Even a fully controlled, honest pair is another person’s scalp. Your own result depends on your donor supply, your pattern of loss and how far it may still progress — none of which appear in a photograph of somebody else.
